We have spent our careers at the point where technology meets the living brain: the operating room.  Between us, we have built the registries that first  measured neurosurgical outcomes at national scale,  the surgical-AI company now used to guide operations  in real time, and the data and computer-vision systems  that are already changing how this work gets done.

From that vantage point, we have watched a new era  arrive faster than the institutions meant to guide it.  Artificial intelligence can now find patterns in  neurological data that were invisible a decade ago.  Sensors and brain-computer interfaces are moving out  of the lab and into real patients. Every one of us has  used tools like these directly, in surgery and in research,  and has seen both what they make possible and what  still stands in their way: fragmented data, absent  standards, and no institution built to hold this work to a  common standard.

The specialty that stands to be reshaped first  by this  transformation is the one we already practice. Data,  standards, ethical frameworks,  a trained workforce, and  capital: the infrastructure this moment requires does not yet exist.

We founded the Foundation for Digital Neurosurgery to build that infrastructure.

Our mission extends beyond neurosurgery. It extends beyond medicine. It extends to the future of human flourishing itself. We believe the coming decades will bring advances that transform how we understand and treat brain health, mental health, learning, resilience, cognition, aging, and recovery–reshaping what is possible for the human mind.

The question before us is not whether this future will arrive. It is whether we will build the institutions capable of guiding it wisely, safely, and for the benefit of all.
